Sorry to hear you're having problems.
That's an issue that I haven't heard about yet.
I assume the problem with the bolt rotating in the lug isn't due to being dirty/build up. There isn't any deformation or burrs where it would cause issue?
Do you have another example to compare the parts side by side?
